What Is the Rotator Cuff?

The rotator cuff is a group of four muscles and tendons — supraspinatus, infraspinatus, teres minor and subscapularis — that surround the shoulder joint and keep the upper arm bone (humerus) stable within the shoulder socket. Together, these muscles allow controlled arm rotation and overhead movement while keeping the joint centered during activity.

What Is a Supraspinatus Tear?

The supraspinatus is the most commonly injured tendon of the rotator cuff. It runs along the top of the shoulder and is the primary muscle responsible for initiating arm elevation. Because of its position and the repeated mechanical load it carries during overhead motion, it is more prone to wear, impingement and tearing than the other three rotator cuff muscles.

Tears range from partial-thickness (a portion of the tendon fibers is damaged) to full-thickness (the tendon is completely torn from the bone).


What Causes a Rotator Cuff Tear?

Rotator cuff and supraspinatus tears generally result from one of three mechanisms:

  • Repetitive overhead activity — common in sports such as badminton, cricket (bowling and throwing), swimming, and weightlifting, where the tendon undergoes repeated mechanical stress.

  • Acute trauma — a fall onto an outstretched arm, a direct blow to the shoulder, or a sudden heavy lifting motion can cause an immediate tear.

  • Degenerative wear — tendon quality naturally declines with age, and the risk of spontaneous or minor-trauma tears rises significantly after age 40, even in people without a sports history.


Symptoms of a Supraspinatus Tear

The most frequently reported symptoms include:

  • Pain when lifting the arm overhead, particularly between 60–120 degrees of elevation

  • Weakness in shoulder rotation and reduced ability to lift objects away from the body

  • Night pain that disrupts sleep, especially when lying on the affected shoulder

  • A catching, clicking or grinding sensation during shoulder movement

Because these symptoms overlap with a simple muscle strain or shoulder impingement, supraspinatus tears are frequently misdiagnosed or dismissed in the early stages, which can delay appropriate treatment and allow a partial tear to progress to a full-thickness tear.


How Is a Rotator Cuff Tear Diagnosed?

Diagnosis begins with a clinical examination, including specific shoulder strength and impingement tests that help localize which tendon is affected. This is typically followed by imaging:

  • Ultrasound — a quick, low-cost way to visualize tendon integrity and detect fluid or inflammation

  • MRI — provides a detailed view of tear size, location and the quality of the surrounding tendon and muscle, and is generally the preferred imaging method when surgery is being considered


Treatment Options

Treatment depends on tear size, patient age, activity level and symptom duration.

Partial-thickness tears are often managed conservatively, through physiotherapy focused on rotator cuff strengthening, activity modification to reduce overhead strain, and anti-inflammatory treatment to control pain and swelling.

Full-thickness tears, particularly in athletes and physically active individuals, generally require surgical repair, since these tears do not heal on their own and tend to enlarge over time without intervention.

Arthroscopic rotator cuff repair is the standard surgical approach — a minimally invasive, keyhole procedure performed through small incisions using a camera and specialized instruments to reattach the torn tendon to the bone. Compared to traditional open surgery, arthroscopic repair typically involves less postoperative pain, a lower risk of complications, and faster return to daily activity and sport.


When Should You See a Specialist?

Shoulder pain that persists beyond two weeks, worsens with overhead activity, or disrupts sleep should be evaluated by an orthopedic specialist, since early diagnosis significantly improves treatment outcomes and can prevent a partial tear from progressing to a full-thickness tear.
